For Our New Patients-
Our new patient paperwork can be accessed via the link below. If you can print this and fill it out prior to your appointment it will save time and allow the doctor to see you sooner!
Dr. Millard Tierce joined FWAAA in April 2022 after previously practicing locally in Fort Worth since 2013. Dr. Tierce, Dr. Bailey, Dr. Rogers and Dr. Beaty are all currently seeing patients in our office location on south Hulen.
For new and existing FWAAA patients:
You can sign up for our Patient Passport in Elation, our Electronic Medical Records (EMR) System. This will allow you to request prescription refills, ask questions and access information through our EMR system. Any patients interested in enrolling will need to provide a valid email address and cell phone number, and an invitation to sign up will then be sent via email upon request. Once accepted Patient Passport can be accessed via any computer, smartphone or other device with internet access. For those who have enrolled the link to Passport is:

COVID-19 Update for all current and prospective patients:
FWAAA no longer requires facial coverings in the office, though they are optional for patients who choose to wear them. We ask that anyone who has been experiencing fever or a new-onset cough or shortness of breath with recent travel or known exposure to someone with COVID-19 not come into the office. We are no longer requiring facial coverings in the office, but anyone who feels more comfortable wearing one is welcome to continue doing so. Please call the office if there are any specific questions.
Thank you and we look forward to serving you however we can.
