31,2025 after caring for patients at FWAAA

since 1984. Dr. Tierce and Dr. Beaty are

available to continue treating all of Dr. Rogers'

patients who require ongoing care.

For new and existing FWAAA patients:

You can sign up for our Patient Passport in Elation, our Electronic Medical Records (EMR) System. This will allow you to request prescription refills, ask questions and access information through our EMR system. Any patients interested in enrolling will need to provide a valid email address and cell phone number, and an invitation to sign up will then be sent via email upon request. Once accepted Patient Passport can be accessed via any computer, smartphone or other device with internet access. For those who have enrolled the link to Passport is:

Before coming into the office for allergy testing, you must check our medication list to see if you have taken any of the medications prior to your testing. If you have taken any of these listed medications prior to the specific days listed in the packet, we will have to reschedule your appointment.
